What is Geriatric Case Management?

A geriatric case manager is a professional who specializes in assisting older people and their families with long term care arrangements. A geriatric case manager has extensive knowledge about the cost, quality and availability of services within their community.

Our case managers will conduct a care-planning assessment to identify problems and need for services. We will arrange for home care, adult day care and other services as well as offer assistance for placement in assisted living facilities and nursing homes.

We will act as a liaison to families living at a distance, making visits to be sure that things are going well and alerting out-of-town families to any problems.
